For each ancestor, the submitter was asked to provide a surname/given name, a single event year (birth, marriage, or death), and the city, county, province/state, and country in which the event occurred. Following the surname index are the names, addresses, phone/fax numbers and e-mail addresses of those Society members who completed a surname index registration form, giving publication permission.
